A poor prognosis is often associated with the concurrent presence of malignancy in individuals with idiopathic inflammatory myopathies (IIM). Early malignancy prediction is widely considered to be beneficial for enhancing the anticipated outcome. Predictive models, in the context of IIM, have garnered limited attention in the literature. Employing a machine learning (ML) algorithm, we sought to identify and predict possible risk factors for malignancy in IIM patients.
A retrospective review of medical records at Shantou Central Hospital, including data from 168 patients diagnosed with IIM during the period of 2013 through 2021, was performed. Patients were randomly divided into two sets—a training dataset of 70% used to construct the prediction model, and a validation dataset of 30% used to evaluate the model's performance. We developed six machine learning models, and their performance was assessed using the area under the receiver operating characteristic (ROC) curve. Last but not least, we developed a web version, featuring the optimal prediction model, for broader application.
A multivariate regression model indicated age, ALT levels below 80 U/L, and anti-TIF1- as factors increasing risk for the prediction model. The study conversely noted interstitial lung disease (ILD) as a protective factor. Evaluating logistic regression (LR) against five other machine learning models revealed its comparable or superior performance in predicting malignancy in patients with IIM. We sought to comprehensively describe the clinical features, disease trajectory, treatment approaches, and mortality outcomes in IIM patients. An effort was made to pinpoint mortality determinants in IIM, and we have investigated.
A single-center, retrospective investigation looked at IIM patients who were determined to meet the Bohan and Peter criteria. Categorizing patients revealed six distinct groups: adult-onset polymyositis (APM), adult-onset dermatomyositis (ADM), juvenile-onset dermatomyositis, overlap myositis (OM), cancer-associated myositis, and antisynthetase syndrome. Sociodemographic characteristics, clinical presentations, immunological assessments, treatment approaches, and causes of death were all meticulously recorded. Using Kaplan-Meier survival curves and Cox proportional hazards regression, a study of survival analysis and mortality predictors was performed.
A total of 158 patients were observed, exhibiting a mean age at diagnosis of 40.8156 years. A large percentage (772%) of patients identified as female and another large percentage (639%) identified as Caucasian. The diagnoses occurring most frequently were ADM (354%), OM (209%), and APM (247%), according to the recorded data. A substantial proportion of patients (741%) underwent therapy using a combination of steroids and one to three immunosuppressive drugs.
