While previous research has investigated the persistence of golimumab (GLM) therapy in Japanese individuals with rheumatoid arthritis (RA), longitudinal real-world observations regarding its long-term use are currently limited. A Japanese real-world study examined the lasting use of GLM in patients with rheumatoid arthritis (RA), considering the influencing factors and the impact of previous medications on treatment persistence.
A retrospective cohort study, employing data from a Japanese hospital insurance claims database, examines rheumatoid arthritis patients. Identified patients were grouped according to their prior treatment: a GLM-only regimen (naive), a single bDMARD/JAK inhibitor treatment prior to GLM [switch(1)], and at least two bDMARDs/JAKs prior to GLM treatment [switch(2)] . Descriptive statistical techniques were used to analyze patient characteristics. Persistence of GLM at 1, 3, 5, and 7 years, and the corresponding factors, were analyzed utilizing Kaplan-Meier survival and Cox regression approaches. The log-rank test was employed to analyze treatment variations.
Persistence of GLM in the naive group stood at 588%, 321%, 214%, and 114% after 1, 3, 5, and 7 years, respectively. The naive group's overall persistence rates surpassed those of the switch groups. Among individuals aged 61-75, and those receiving concurrent methotrexate (MTX) treatment, a greater degree of GLM persistence was apparent. Compared to men, women experienced a lower rate of treatment abandonment. Factors such as a higher Charlson Comorbidity Index, an initial GLM dose of 100mg, and switching from bDMARDs/JAK inhibitor regimens were predictive of a lower persistence with treatment. Subsequent GLM persistence was longest with the prior medication infliximab. Tocilizumab, sarilumab, and tofacitinib displayed significantly reduced persistence durations, respectively, with p-values of 0.0001, 0.0025, and 0.0041, reflecting the comparative analysis.
